How Healing Actually Works
When an implant is placed, the body responds in two parallel tracks. The soft tissue (gum) closes over days. The bone, however, undergoes osseointegration, a slow remodelling in which living bone grows into contact with the implant surface and begins to share load. This is the foundation of long-term functional stability, and it cannot be rushed.
This is why we document implant torque above 35 Ncm at placement and confirm an ISQ stability reading of at least 68 before loading the implant with a final restoration. Those are measurable thresholds, not estimates. The timeline below describes the typical biological course; your individual healing will vary with your bone quality, your medical history, and whether grafting was part of the plan.
A note on sensation: during healing you may feel pressure, mild throbbing, or tightness as tissue remodels. Pressure and vibration are mechanical sensations, not pain. Real Risks: Inflammation vs Infection, and When to Call
Healing is biological and varies between people. No surgical procedure carries zero risk, and we will not tell you otherwise. The complications below are uncommon, but you should know them, why they occur, and how they are monitored.
Inflammation is expected. It is the body's controlled response to surgery: swelling, warmth, and tenderness that *peaks early and then steadily improves*. The direction of travel, getting better, is the reassuring signal.
Infection is different. Watch for the reverse pattern: pain or swelling that *worsens after day three*, spreading redness, a bad taste or discharge, or fever. Infection means bacteria have taken hold and need clinical intervention; it does not resolve on its own.
Other complications we monitor for and manage:
- Failed osseointegration, the implant does not bond adequately to bone (more likely with poor bone quality, smoking, or uncontrolled diabetes). This is why we measure stability before loading rather than after.
- Excessive or prolonged bleeding, minor oozing in the first day is normal; persistent active bleeding is not.
- Altered sensation, temporary numbness or tingling near the site can occur as tissues settle; persistent altered sensation should be reviewed.
- Sinus involvement (upper jaw, near the sinus floor), assessed in advance with CBCT imaging precisely to reduce this risk.
