When It Is Indicated, and When It Is Not
Whether same-day teeth are appropriate is decided at surgery, against measured criteria. The four states:
- Indicated when implants reach strong primary stability, insertion torque documented above 35 Ncm, and bone quality supports immediate, controlled load distribution. The provisional can be splinted across the arch and loaded safely.
- Optional when stability is borderline. Immediate loading *may* be possible but carries a higher chance of disturbing integration; a short conventional delay is the more conservative path. The choice is discussed against that trade-off.
- Urgent, not applicable. Immediate loading is a convenience-and-function protocol, never an emergency. Disease control may be urgent; loading on day one never is.

Why discomfort occurs here. Placing implants involves preparing sites in bone and lifting gum tissue, both densely innervated. The nervous system reports this controlled wound as pain. Same-day cases often involve several implants, so the healing response is across a wider area.
How anaesthesia blocks it. Local anaesthetic blocks the sodium channels in the sensory nerves serving the surgical area, so pain signals never reach the brain, the block is at the nerve. Sedation is commonly added for full-arch same-day work so the patient stays comfortable through a longer procedure.
What you actually feel. With pain blocked, you may still feel pressure and vibration as the implants are placed, these travel on separate nerves from pain. That awareness is expected; sharp pain is not and is addressed at once with more anaesthetic.
Normal recovery vs infection. Expect swelling, bruising, and soreness that peak in the first 2–3 days, then ease. That is normal inflammation. Pain or swelling that *worsens* after day three, with fever or discharge, follows the pattern of infection rather than healing.
When pain signals a complication. With a freshly loaded implant, pain that intensifies, looseness or movement of the provisional, or persistent numbness should be reported promptly, these can indicate the implant is being disturbed during integration.
